Guidance for service information template

When you fill in the form there is guidance and example text in the template.

You will need to fill in one form for each service.

Each service page can only have one registered email address used to login.

We are

Use this section to introduce your service. Do not worry too much about the details of what you do in terms of counselling, therapy etc or criteria. We will ask you about that later on.

Just a quick overview to tell users who you are. There is an example on the template.

We work in

This section is where your service operates. This could be nations, regions, cities or postcodes.

We offer support with

Use this section to give users a little more detail.

You will get to select from a list of options, but if your area is not listed, there is the option to add it yourself.

Try and limit it to a word or two.

We provide support using

This section is where you can tell users what support you offer. You will select from a list of options.

You can also add in your own text, if the support is not listed.

Try and limit it to a word or two.

We help ages

You can tell users what ages you help by selecting as many options as you need.

There is also the option to add in any other ages not listed.

Get help from our service

This section tells the user how to access your service.

It might be that they need a GP referral, or from a social worker, or they can self-refer. This gives the user context.

Examples:

  • “Your GP needs to refer you. They can fill in our form online.”
  • “You can refer yourself to our service. Download, fill in our form, and email it back to us.”

Keep sentences short and to the point.

If you do not accept referrals through your service page, then you do not have to any add referral forms or include any referral links.

Add a form

You’ll be asked if you refer using a form users can download or a form they can fill in online.

Select the one relevant to your service. You can then add the link or upload any forms you have.

There are options to select that will allow users to submit a:

  • self-referral
  • professional referral
  • parent referral
  • enquiry form

When writing the link text for the form (this is the content that will appear on the page) it is important that the pages are consistent.

For any form you add, please use the following link text where relevant:

  • young person looking for help (online) 
  • young person looking for help (download) 
  • professional wanting to refer a young person (online)
  • professional wanting to refer a young person (download)
  • parent or carer looking to get help for a young person (online)
  • parent or carer looking to get help for a young person (download)
  • Fill in our enquiry form

Contact us

Any information here will be displayed on the website.

You can leave out any information that you do not want to be displayed. For example, if you deal with domestic violence. Only enter the information appropriate to share such as the website address.

Our funding

You can add the name of any organisation that funds you. This might be a condition of the funding, or because you want to show who helps fund you.

You can also upload a logo of your funder.

Adding images

You have the option to add an image to your service page. It can be a:

  • header image – this sits across the top of the page
  • teaser image – this appears on the service listing page and is a smaller image

If you upload a header image, you should also use this as the teaser image.

Image size

Your image needs to be high resolution, in JPEG format, and:

  • header image - 1512 pixels (width) x 850 pixels (height)
  • teaser image – 720 pixels (width) x 404 pixels (height)
  • logo image – 768 pixels (width) x 432 pixels (height)

To resize your logo to meet the required dimensions, you can use a free image resizing tool.

Your image cannot have:

  • text in it (added on to the image)
  • logos visible

It also needs to have permissions and consent including from:

  • the owner (if it is from the internet or you didn’t take it yourself)
  • any young people (if you took it yourself)

Uploading documents

You can upload documents if you think they will be helpful. Try not to add any that:

  • repeat the information on the page
  • are hard to read
  • you would send out later

The document will appear on your page using the title you have saved it as. Try to name your document in a way that describes what it is. For example:

  • ways we can help with your mental health
  • websites you can visit whilst you are waiting
  • 10 things you can do to help yourself

Avoid titles that use odd characters or are internally facing.
